Probably either a worn or broken gear on the print head drive, (the yellow ribbon) a print head misalignment, or a bent/dented rail. It could also be something scrambled from a power surge, lightning strike, etc. So you might try running a print head alignment, try to tell it to update firmware, check for software updates.
